Answer: Ah, so close @GregYagoda! These three Polaris jams are the 7/15/00 "Piper," the 7/31/98 "Ya Mar," and TAB's 7/24/01 "Simple Twist Up Dave" - hence the part in the OP about naming the band, and the missing band name on the results spreadsheet for jam #3. The blog wins for only the 28th time ever! Next week, we're playing for two codes...

I think "the band" was just a red herring. They're all Phish.
I did notice “the band” hint and the TAB shows at Polaris but couldn’t find audio on Relisten so ran out of time and gave up. This was fun—will try again next week!
I’d never throw just a random TAB clip up by itself, but in a themed MJM / last of month with a pre-hint? I’d argue it’s not a complete dick move (just a little bit haha)
